Autumnwood Care Center responds to the changing health care needs of our community by offering quality long-term care, memory support and short-term rehabilitation delivered by a specialist-led team of licensed nurses, rehab therapists and social service professionals.

Care Center
Residents of our long-term care center are thoughtfully cared for physically, emotionally, socially and spiritually. We create a family atmosphere so each resident feels welcome, comfortable and valued.

Short-term Rehabilitation
Choose Autumnwood's specialist-led short-term rehab on your journey toward better health, and you'll find what seems like a challenge becomes our shared goal: attaining your highest levels of health and independence, safely and successfully.

Memory Support
Our specialized memory support program provides a tranquil household where people with memory loss experience a structured schedule of daily care and activities while having their health care needs met — all in an environment with the everyday comforts of home.

End-of-life Care
We recognize that loss and anxiety accompany the end of life and are shared by both individuals with terminal illness and their loved ones. At Autumnwood, we provide medical, emotional and spiritual assistance to people facing the end of life, in a comfortable and familiar place, often in partnership with hospice programs. Each member of our staff is specially trained to provide compassionate, attentive end-of-life care.

Respite Program
For short stays, respite guests enjoy the same short-term rehab, long-term care and memory support health care and services Autumnwood residents receive, including individualized plans of care, amenities and an array of social, recreational and spiritual activities.
