Autumnwood Care Center, a 115-accommodations, skilled-care facility, has offered premier quality skilled nursing and rehabilitation services since 1977. We care for individuals with chronic diseases and for those recovering from injury or illness. Our residents receive 24-hour care from well trained, dedicated professional staff. Because we have the ability to coordinate complex medical care regimens residents receive prompt, attentive care within our modern home-like setting. We are a one-floor facility with large, private rooms, as well as many shared rooms.

We provide a secured area named Freedom Hall to serve the needs of those with Alzheimer's and dementia. Specially trained staff work closely with family members and other professionals to be sure each of our residents on Freedom Hall have the opportunity to enjoy their life at this difficult time.

We also provide a special care area called The Compassionate Care Suite which serves the population who are in need of hospice services. We contract with Heartland Hospice, Community Hospice and Bridge Hospice who help provide end of life services for those residents who currently reside here or for others who need hospice care and cannot remain in their own homes. Families are welcomed to stay with their loved ones in this area.

Our Rehabilitation Department features some of the best therapy professionals in our area. Whether it is for a new admission who needs to regain strength after orthopedic surgery or for those who need re-training after a debilitating injury, our occupational and physical therapists work closely with the physicians to achieve maximum improvment in the shortest period of time. Many of our rehabilitation patients are able to return to a fully functional lifestyle after a brief stay in our facility. Our current building improvements will offer larger private rooms with many added features such as microwaves, refrigerators, wall mounted tvs and comfortable lounge chairs for those residents who will be residing at Autumnwood for a brief time during their therapy sessions.

We recognize that individuals have unique physical, emotional and spiritual needs. Our staff understands that the living environment influences health and mental outlook of our residents. Families and friends are welcome at any time and are encouraged to be involved with their loved one's care and activities.
