Rehabilitation Services


The Montrose Rehabilitation Center is located at Valley Manor Care Center at 1401 S. Cascade Ave, Montrose, Colorado 81401. Phone # 970-252-3107, and Fax # 970-249-8510.

Our rehabiliation unit has just been remodeled and has 13 private rooms with private baths, 1 semi private room and a transitional apartment.Our transitional apartment simulates the activities of home. This allows our therapy staff to explore strategies prior to home discharge. This unit has wireless internet, flat screen TVs and dining room just for our guest that has family style dining for lunch and dinner.

Guests can enjoy the accommodations in our skilled rehabilitation and nursing unit, dedicated to maximizing the functional skills of the individuals that expect to return to an active lefstyle in the community. discharge to the community is our primary goal. To achieve this, we will work with you and your physician to establish appropriate goals.

Diane Shipley RN is our nursing manager for this unit and Lynette Gilbert OTR is the Director of Rehabilitation.

Upon discharge from our unit, if you still have some unmet needs that can be provided in your home, Volunteers of America also have a home health agency to assist with your needs. Volunteers of America Home Health of Western Colorado serves clients in Ouray, Ridgway, Montrose, Olathe, Delta and Mesa Counties. You can reach them at 970-240-0139.

Outpatient treatment is available at our facility. We have Comprehensive Rehabilitation with Physical Therapy, Occupational Therapy, Speech Therapy, Dietary Consultation, and Social Services. Some of our treament areas include:

Chronic Neurological Disease, Vestibular Rehabilitation, Heart and Lung Disease, Joint Replacements, Lymphedema Management, Soft Tissue Injuries, Stroke, Worker's Compensation and fall Prevention Programs.
