Assisted Living
Assisted Living Level 5. Services included in Assisted Living Level 5 include:  three meals per day, all utilities excluding personal telephone and enhanced basic cable television.  Emergency call system, RN supervision, medical assessments and service coordination, 24-hour caregiver assistance, social and recreational events and activities.  Weekly housekeeping, weekly linen changes, weekly linen and personal laundry, medication management/set up and administration 4 times or more a day. Assist to and from meals and activities, Staff assist with bathing/shower 2 times or more per week.  Assist with dressing and grooming, transfer with staff assist of 2.  Dependence in incontinence management.  Injections, i.e., B-12, nebulizer treatments.

The Assisted Living lifestyle is about choices.  Choose when and how you desire to receive services. Choose from our cozy, furnished settings that look and feel like home.  Our Assisted Living has twenty-eight suites.  Twenty-two are private suties, four are shared suites and two are one-bedroom suites all in our one story building.  We offer five levels of care to meet the needs of our residents.  Our levels of care include the rent of the suite and the personal care services.

Memory Care

The Homestead at Coon Rapids Memory Care Program is devoted to caring for individuals with Alzheimer's Disease or other memory impairments. Our Memory Care has twenty-four suites.  Twenty-two are private, cozy furnished suites that look and feel like home and two are shared suites.  We offer three levels of care to meet the needs of our residents.  Our levels of care include the rent of the suite and the personal care services.

Confident Choices Award
Confident Choices designees must meet requirements that have been developed by experienced housing-with-services providers and are based on standards of practice that truly influence the quality of a consumer's experience. Confident Choices designees must go beyond basic compliance with state and federal requirements and must have implemented specific elements that are important components of quality housing-with-services.  To receive the Confident Choices designation, a senior building must complete a detailed application describing a variety of programs and initiatives and must pass a site review by two experienced housing professionals not affiliated with the building.  This site review is designed to verify that the provider meets the high standards of the Confident Choices program.  The Confident Choices designation must be renewed every two years so that consumers can be assured that the building continues to be an excellent senior housing option.
