About Us
For many years Horizons Health Care and Retirement Community has provided quality skilled nursing care and rehabilitation services to our community. Our long-standing reputation is an assurance that you can trust the care of your loved one to us. Our philosophy embodies dignity, quality of life, and respect for the individual. We care for our resident's physical condition and promote their spiritual, emotional, and social well-being.
Horizons Rehabilitation Center is also located on our campus. We are able to provide Physical Therapy, Occupational Therapy, Speech Therapy, Dietary Consultation, and Social Services through our Rehabilitation Center.
Some of our treatment areas include:Chronic Neurological Disease, Vestibular Rehabilitation, Heart and Lung Disease, Joint Disease, Joint Replacements, Lymphedema Management, Soft Tissue Injuries, Stroke, Worker's Compensation, and Fall Prevention Program.
In addition to our Health Care Center our campus features Horizon's Retirement Community, with a senior mobile home park and independent senior apartments. All of our residents are given priority access to all our living settings. Our complete continuum of care can meet all the needs of the seniors.
