Horizons Health Care Center has been serving the community since 1972 and provides quality skilled nursing care and inpatient/outpatient rehabilitation. It was founded by a local physician for the local community. Horizons Health Care is an integral part of the Surface Creek community of Cory, Austin, Eckert and Cedaredge. We are located in the shadows of the Grand Mesa on nine acres in rural western slopes of Colorado. We care for individuals with chronic diseases and for those recovering from injury or illness. We have an 11-bed rehab wing with private and semi-private rooms dedicated to short term rehab residents with our goal to enable them to return to their home. We also have a 16 bed Memory Care Unit for the care of individuals with Alzheimer's and other dementia type illness who wander and/or need quiet structured environment. Our staff provides kind, compassionate care to our 'family' and we welcome families and friends to be involved with their loved one's care and activities.

Horizons Health Care Center is a 78 bed skilled care facility located in Delta County about 13 miles from the city of Delta. It's scenic rural setting provides a view of the Grand Mesa and the feel of small town country living. The facility was built in 1972 and has added an 11-bed rehab wing in addition to a state of the art rehab gym.The facility is on nine acres of rural property and beautifully landscaped lawns. We also have a newly designed 'Courtyard Garden' with raised planters where our residents grow vegetable and flowers for their own eating a pleasure. Residents enjoy their outdoor oasis with the sound of water fountain, buzzing of hummingbirds, sweet smell of flowers and companionship of family and friends.

Horizon Rehabilitation Center offers specialized therapy care. We have 4 private rooms, 3 semi-private rooms, and a transitional room that simulates the activities of home. We offer on site beauty and barber services, transportation in our 2 handicapped accessible van, comfortable and homey gathering areas and opportunities to participate in community events. Our activity room has a large screen TV and we have an activity kitchen for those who like to cook.
