The Admission Process
Dear Resident, Family Member or Concerned Person,
Our mission is to promote independence and to help preserve the dignity of the elderly. We welcome your inquiry into our services and residential features. To schedule a personalized tour of our facility, or if you would have any additional questions, please contact our admissions department at (952) 925-8543.
The first step toward entering Edina Care and Rehabilitation Center is to contact the Admission Coordinator (952)925-8543 for information regarding bed availability. We strongly encourage you to arrange a tour of Edina Care and Rehabilitation Center prior to admittance as it is important for family/friends to meet the staff and feel comfortable with the skilled nursing facility you choose for your loved one.
Everyone who is admitted to Edina Care and Rehabilitation Center is assessed prior to admission. If the potential resident is hospitalized, we will request faxed information from the hospital to complete the nursing assessment. If we feel that we need additional information to make our decision, then the Admission Coordinator will request the needed information to assess the potential resident. Upon admission,our staff will request that the hospital send a history and physical, signed physician's orders, signed transfer form, discharge summary, level of care and rehabilitation information with the resident. If the resident is to be admitted from home or another long term care facility the same assessment process applies. You will then be given an envelope with the necessary papers for your primary Doctor to complete within 24 hours of admission.
Once the potential resident has been approved for admission, you will be asked to complete an admission packet. We also require a photo copy of both the front and back of the Medicare card and any other forms of Health Insurance. You will be asked to provide copies of forms if relevant i.e., Living Will, Power of Attorney for Finances and Health Care, Durable Power of Attorney, Conservatorship and Guardianship.
Please read the Philosophy Statement and Admission Policies included on the reverse side of this letter. It is important that you be aware of the information prior to admission.
The Philosophy of Edina Care and Rehabilitation Center is to restore the health and physical functioning of our patients to their maximum potential so they can return to their previous living situation. At Edina Care and Rehabilitation Center, our compassionate staff is dedicated to maintaining a high quality of life with quality care for our residents.
Assistance from Nursing, Administration or our Medical Director is utilized as necessary to determine if Edina Care and Rehabilitation Center can provide the nursing care required by the resident. An applicant is admitted only upon the recommendation of a physician.
Edina Care and Rehabilitation Center will not admit or retain a resident;
A) who would cause harm to self or endanger others;
B) who needs continuous one-to-one supervision;
C) who has an uncontrolled infectious process that staff is not able to manage for reason including but not limited to:1) No private room available, or 2) Individual's presence creates a risk of harm to others residents and individuals;
D) who needs special physical environment adaptations that the Care Center cannot provide;
E) Individual is a registered predatory offender;
F) who with uncontrolled behavior problems would infringe on the rights of other residents and staff;
G) who is unable to utilize available standard equipment, i.e., lifting/mechanical restrictions 300 pounds or greater;
H) who requires services including technology, that exceed the staffing levels and patterns available, including but not limited to,IV therapy treatment,IV chemotherapy, blood transfusion, morphine drips, IV push medications, peritoneal dialysis, residents using ventilator/respirator;
I) who is unable to pay for services;
J) who is chemically dependent;
This is not an inclusive list. Questionable admissions are discussed with the Medical Director, Director of Nursing and Administrator for final approval.
We recommend that you discuss your health care wishes with family and that you complete an advance directive, such as a Minnesota Health Care Declaration, Living Will, Comfort Measures Only, Mental Health Declaration, Durable Power of Attorney and/or Appointment of Conservator or Guardian.
