About Us
Volunteers of America's Autumnwood Care Center has been in operation on State Route 18 in Tiffin since 1977. Offering both long term care and short term rehabilitation in a safe, home like environment has been what has made us the facility of choice in this area. We rely on professional nursing staff who provide their skills 24 hours a day, 7 days a week plus a superb team of therapeutic recreation specialists, housekeepers, laundry personnel and talented dietary staff to make the lives of those residents who choose Autumnwood memorable. There is nothing that makes a place feel more like home than smiles and hugs and that's where we receive excellent marks. Our staff and our residents seem like family for the times they are together. Residents who have received our services and then returned to their homes or who have located to other areas comment on our discharge surveys that the care they received was wonderful and the attitudes of those who cared for them would be the reason they would return to us if they needed to.
We provide a secured area named Freedom Hall to serve the needs of those with Alzheimer's and dementia. Specially trained staff work closely with family members and other professionals to be sure each of our residents on Freedom Hall have the opportunity to enjoy their life at this difficult time.
We also provide a special care area called The Compassionate Care Suite which serves the population who are in need of hospice services. We contract with Heartland Hospice, Community Hospice and Bridge Hospice who help provide end of life services for those residents who currently reside here or for others who need hospice care and cannot remain in their own homes. Families are welcomed to stay with their loved ones in this area.
Our Rehabilitation Department features some of the best therapy professionals in our area. Whether it is for a new admission who needs to regain strength after orthopedic surgery or for those who need re-training after a debilitating injury, our occupational and physical therapists work closely with the physicians to achieve maximum improvment in the shortest period of time. Many of our rehabilitation patients are able to return to a fully functional lifestyle after a brief stay in our facility. Our current building improvements will offer larger private rooms with many added features such as microwaves, refrigerators, wall mounted tvs and comfortable lounge chairs for those residents who will be residing at Autumnwood for a brief time during their therapy sessions.
