Transitional Care
Our philosophy is "to restore the health and physical functioning of our patients to their maximum potential, so they can return to their previous living situation." The entire second floor is dedicated to transitional care, with 43 beds in both private and semi-private rooms and has convenient 24-hour admission, seven days a week. Other features include:
Amenities: Each room has accessible complimentary satellite TV and telephone services. Full meal service and snacks are provided daily. Access to lounges with internet access.patio areas, and our pet therapy program offer comfort and relaxation for patients and their visitors. Visiting hours are unrestricted.
Health Facility Services: Physical, Occupational, Speech and Language Pathology services. Our in-house multi-disciplinary therapy staff consistently receive nearly 100% satisfaction ratings on surveys from patients leaving transitional care. Individualized therapy services take place 7 days a week in several spacious rehabilitation clinics, nestled in our charming "town square" setting. After discharge, the option to return to our clinic on an outpatient basis and continue working with therapists is available.
24 Hour Skilled Nursing Services. Our nurses have the training, skills and experience to provide complex medical care and management of multiple medical problems, such as orthopedic and neurological diagnosis complicated by diabetes and heart conditions. Common services include wound care, pain management, oxygen, infusion therapy, suctioning, nutritional support and post-surgical care.
Discharge Planning. Full-time social workers will work with you to coordinate your discharge. Together with the entire rehab team, they help to identify the equipment and services that will safely support seniors in their home, and then make certain that all arrangements are in place before discharge.
